Academic Detailing Service

Jana is the CEP academic detailer for northwestern Ontario and the Ottawa region. She has broad experience in pharmacotherapy and clinical evidence appraisal with expertise in chronic pain and safe opioid management in primary and community care. 

Mathew is the CEP academic detailer for northeastern Ontario and the Waterloo-Wellington region.A pharmacist for 15+ years with a background in pharmacy research, his tailored detailing sessions support primary care clinicians in applying the principle of evidence medicine to clinical practice. 

Nicole is the CEP academic detailer for southwestern Ontario and the Erie St. Clair region. She has clinical experience in mental health, nephrology, and geriatrics, and is passionate about sharing practical applications of evidence to advance patient care.

Sachin is the CEP academic detailer for southeastern Ontario and the Mississauga-Halton region. A clinical pharmacist and consultant, he specializes in working with family physicians to optimize treatment plans across a variety of practice settings.  

Silvana is the CEP academic detailer for the Hamilton-Niagara-Haldimand-Brant region. A pharmacist for 25+ years, her extensive knowledge of pharmacotherapy and adaptivity ensure academic detailing visits are tailored to the expressed needs of each clinician.  

Trish Rawn is the CEP academic detailer for Toronto and the North Simcoe Muskoka region, and the clinical service director for CEP's Academic Detailing Service. Trish is highly skilled in critical literature appraisal, academic detailer training, tool development, and interprofessional relationship building.

Janie Bowles-Jordan works as a clinical pharmacist and academic detailer with the Hamilton Family Health team and Compass Community Health Centre. She works with the chronic disease states of diabetes, pain, mental health, geriatrics, palliative care and smoking cessation. As the regional clinical coordinator with the University of Waterloo School of Pharmacy, she also placed, oversees and mentors students' experiential rotations in the Hamilton region.

Janie obtained her degree from the University of Toronto. She completed her hospital residency and worked previously at St. Joseph's Hospital in Hamilton.

Leslie Bhardwaj is a clinical pharmacist, academic detailer, and professional practice lead for pharmacy with the Hamilton Family Health Team. She works in a multi-disciplinary primary care team setting where her practice involves chronic disease management including diabetes, cardiovascular disease, smoking cessation and chronic pain.

Leslie obtained both her pharmacy degree and Doctor of Pharmacy from the University of Toronto.

Lucy Feng is a clinical pharmacist and academic detailer at the Hamilton Family Health Team. She has also worked as a nephrology pharmacist at London Health Science Center where she completed her hospital pharmacy residency.

Lucy obtained her pharmacy degree and Doctor of Pharmacy from the University of Waterloo.

Peter Carducci is a clinical pharmacist and academic detailer at the Hamilton Family Health Team. While working in primary care he receives consults to optimize medication management for patients with chronic disease. He works alongside a multi-disciplinary team to provide pharmaceutical care for patients across Hamilton.

Peter obtained his Doctor of Pharmacy degree from the University of Waterloo in 2020. Peter completed a hospital pharmacy residency at Trillium Health Partners prior to joining the Hamilton Family Health Team in 2021.

Dr. Margaret Jin is currently working with the Health For All Family Health Team as a clinical pharmacist and certified diabetes educator. She has worked in many pharmacy settings including community and teaching hospitals, community pharmacies, nursing homes and primary care. 

Margaret received the bachelor of science pharmacy (BSc PHM) and doctor of pharmacy (PharmD) degrees at the University of Toronto; and the Health Research Methodology Master of Science degree at McMaster University.

Janice Hall is a pharmacist with the Prince Edward Family Health Team. She provides pharmacy services in a multidisciplinary environment, with an emphasis on complex patients with chronic diseases, smoking cessation and participation in a primary care memory clinic. Janice has held positions in various hospital, community and correctional services pharmacies before moving to primary care in 2008. 

Janice obtained her pharmacy degree from the University of Toronto, and is currently enrolled in the Master of Health Professions Education program at Queen’s University. 

Dennis Angevine is a clinical pharmacist and academic detailer at the Thames Valley Family Health Team. He works in a multi-disciplinary team with a focus on geriatric medication reviews, diabetes, COPD, and smoking cessation. He has previously worked as both an inpatient and outpatient pharmacist at the London Health Science Centre.

Dennis obtained his pharmacy degree from Dalhousie University.

Katherine Koroluk is a clinical pharmacist currently practicing at the Thames Valley Family Health Team in London, Ontario. She has worked in a broad range of clinical settings including primary care, home care, inpatient medicine, and community pharmacy. She has a special interest in polypharmacy and deprescribing, home care, and shared decision making surrounding medication use.

Katherine has completed all of her training at the University of Toronto. Her degrees include a Bachelor of Science and Doctor of Pharmacy, and she is in the final stages of completing her Master of Science. She has also completed a primary care pharmacy residency at the University Health Network in Toronto.

Payal Patel works with the Thames Valley Family Health Team as a clinical pharmacist and clinical lead. She has an extensive history in primary care, hospital care, emergency medicine, as well as academia. At present, she has a particular interest in deprescribing and helping  patients make informed decisions about their drug therapy.

Payal obtained her pharmacy degree form the University of Manitoba, completed her hospital residency at the Alberta Childrens Hospital, and received her Doctor of Pharmacy degree from the University of British Columbia.

William Ong is a clinical pharmacist currently practicing at the Thames Valley Family Health Team as part of a multidisciplinary team. He has 20 years of retail pharmacy experience. Prior to completing his pharmacy degree he worked as a hospital pharmacy technician. In the past he has been an instructor at Fanshawe College (London, ON) and worked with the Pharmacy Examining Board of Canada. He is currently involved with working and teaching medical residents at the St. Josephs Family Health and Dental Centre (London, ON). He has a special interest in smoking cessation, the pharmacist/patient interaction, and diabetes care. 

William obtained his pharmacy degree from the University of Saskatchewan.

Dr. Arun Radhakrishnan is a family physician with a focused clinical practice in chronic pain. He is an Assistant Professor with the Department of Family Medicine at the University of Ottawa and the University of Toronto. He is affiliated with The Ottawa Hospital and the Elisabeth Bruyere hospital and is an investigator at the Bruyere Research Institute and the Equity in Health Systems Lab. His research and education interests are in knowledge translation mentoring and digital health technologies. He has led the development of several award-winning educational programs, clinical practice tools and resources to support primary care management of chronic pain. He is a past AMS Phoenix Fellow and is currently the Medical Director of the Adaptive Mentoring Networks with the Centre for Effective Practice.

Dr. Radhakrishnan provided clinical leadership for the development of the chronic non-cancer pain topic.

Dr. Felicia Presenza is a care of the elderly physician working in Sudbury, ON. She works at St. Joseph’s Continuing Care Centre on the rehab floor, and also provides dementia care at memory clinics. She has a geriatric-focused family practice and is faculty with The Review Course helping family medicine residents prepare for their CCFP exam.

Dr. Presenza provided clinical leadership for the benzodiazepine use in older adults topic and the fall prevention and management topic and was offered compensation for her role. 

Dr. Rahul Jain is an academic family physician and hospitalist at Sunnybrook Health Sciences Centre. He is an Assistant Professor in the Department of Family and Community Medicine at the University of Toronto. Dr. Jain is involved with heart failure guideline development (C-CHANGE and CCS), knowledge translation activities including presenting at national conferences, co-leading an update of the outpatient HF flowsheet to guide clinicians in practice, invited to speak at the HeartLife Foundation conference for patients/caregivers, and volunteers with the Heart and Stroke Foundation as a spokesperson on media campaigns and on roundtable discussions to build a national action plan for heart failure.

Dr. Jain provided clinical leadership for the heart failure topic.

Dr. Risa Bordman is a family physician and palliative care clinician in Toronto. She has extensive experience in research and education and is an Associate Professor at The University of Toronto and McMaster University. She is the Residency Program Director at the Foundation for Medical Practice Education, Faculty Development lead for the Office of Education Scholarship at the University of Toronto and Co-chair Palliative Care Education Committee, Division of Palliative Care, University of Toronto.

Dr. Bordman provided clinical leadership for the type 2 diabetes topics and was offered compensation for her role. 

Dr. Sharon Bal is a family physician practicing in Cambridge with privileges at Cambridge Memorial Hospital. She is a Regional Clinical Lead (Ontario Health, West) and an Assistant Clinical Professor in the Department of Family Medicine, Michael G. DeGroote School of Medicine, McMaster University, where she holds leadership roles as Preclerkship Coordinator (Waterloo Regional Campus) and Chair, Electives of the UGME Program. Dr. Bal has a special interest in health equity and mental health, chairing the CND Quality Committee and participating in provincial initiatives including the Advisory Committee for the Anxiety Disorder Quality Standard (HQO), and is a current member of the Depression and Anxiety-Related Disorders Provincial Advisory Table, OH.

Dr. Bal provided clinical leadership for the anxiety and depression topic and was offered compensation for her role.

Dr. Winyan Chung is a family physician in Sault Ste. Marie ON, working in primary care, long term care and hospital in-patient service. She is currently working as clinical co-lead for the Healthy Aging Project with the Algoma Ontario Health Team.

Dr. Chung provided clinical leadership for the fall prevention and management topic and was offered compensation for her role.
