Chronic diseases require ongoing medical care and are leading cause of death in Ontario. This section consists of a range of tools , clinical decision supports and resources to address the broad needs of providers treating those living with chronic diseases (e.g., diabetes, insomnia, osteoarthritis, etc.).

The demand for services and care to meet the needs of aging individuals will continue to increase over the next 10 to 20 years. This section contains tools , clinical decision supports and resources applicable to and specific to those 55+. It includes fall prevention, osteoarthritis & more.

One in five adults report chronic pain (pain experienced every day for 3 or more months). This section includes chronic non-cancer pain, low back pain, neck pain and headache, along with osteoarthritis.

OHTs can improve the patient experience and strengthen care in the community for populations with specialized needs. This section consists of medical assistance in dying (MAID), childhood obesity, poverty & more.

These virtual care resources & tools with EMR integrations can help you provide the best possible outcomes for patients. Topics include chronic non-cancer pain, preconception etc.
