Intended for clinicians who are participating in E2P or using E2P tools and supports, this list provides easy access to local community and self-management resources that meet the individual needs of patients.

Resources have been curated for specific health conditions as part of the E2P program. The current list of resources has been curated for persons living with heart failure. Additional resources for other health conditions will be added as tools are developed.

If you are a person living with heart failure, please contact your primary care provider to discuss community referrals. If you do not have a primary care provider, please contact Health Care Connect Ontario.

6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF). Equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily. Weekly discussions with telehomecare nurse.
