Cardiac Rehabilitation and Education Program

OTMHMDH
The Cardiac Rehabilitation Program is an individualized exercise and educational program, designed to improve cardiovascular fitness and reduce risk factors for coronary artery disease. This program:
  • is most beneficial to people who have had some type of heart-related event including: angina, heart attack, congestive heart failure, heart surgery, or angioplasty.
  • includes medical evaluation, graded exercise testing, individually prescribed exercise programs, supervised exercise classes, risk factor education, and counselling.
  • is run by a multidisciplinary team including a medical director, administrative assistant, nurses, physiotherapists, dietitian, kinesiologists, social worker, and volunteers.
  • is offered at both Oakville and Milton hospitals.
  • provides education and activity guidelines for patients to utilize between hospital discharge and beginning the outpatient program.
To participate in the Cardiac Rehabilitation Program, a Physician Referral is required.

Program Overview

Milton District Hospital (MDH)

The Cardiac Rehabilitation program at the MDH operates on Mondays and Wednesdays from 10:00 a.m. to 6:00 p.m. Patients attend once per week for six months and participate in supervised exercise and educational programs.

Oakville Trafalgar Memorial Hospital (OTMH)

The program operates on Tuesdays and Thursdays from 9:00 a.m. to 7:00 p.m. Patients attend once per week for 6 months and participate in supervised exercise and educational programs.
