Introduction
During pandemics, historically marginalized and minoritized individuals suffer disproportionately. We developed this resource, in partnership with Upstream Lab and the Department of Family and Community Medicine at the University of Toronto, to help health and social care providers support individuals exposed to COVID-19 related social risks that impact health.

About the tool
This resource is part of the CEP's COVID-19 Resource Centre. For each of the resources, the CEP actively pulls together, rethinks and translates the latest information so family physicians and primary care nurse practitioners can make confident decisions in their practice.
