With the emergence of new medications with weight loss and cardiovascular benefits as well as biosimiliar insulins, clinicians need easier point-of-care access to evidence-based tools to support more personalized management and improve patient health outcomes.

Acute Care
- Support clinicians in identifying those with uncontrolled or elevated blood sugars to help patients receive appropriate referrals and education.
- Support hospitals in giving accurate and helpful information to patients and primary care clinicians through improved discharge summaries, helping patients transition from the hospital to the community.
- Embed quality standards into hospital health information systems (HIS) to help clinicians deliver quality care to their patient populations.
- Offer hospital sites project management support, clinical & informatics leadership, and funding to support the hospital in embedding the quality standard into their workflows.

- Assist in screening for risk factors and diagnosing prediabetes and type 2 diabetes using standardized testing.
- Help clinicians connect patients to up-to-date and relevant resources on the prevention and management of prediabetes and type 2 diabetes.
- Streamlined navigation of available services and supports, tailored to individual patient needs.
- Embed best practice quality standards into electronic medical records (EMRs) in modular format, allowing tailoring and alignment with clinical workflows.
- Available for OSCAR Pro and coming soon for Telus PS Suite EMR.
