Sickle Cell Disease (SCD) is a priority topic and a new use case within the E2P program for acute care hospitals. Ongoing scoping refinement is occurring with Ontario Health and existing provincial structures based on the Sickle Cell Quality Standard and Implementation Toolkit. This can include (but is not limited to):
- Provide training resources that support team members to have the knowledge, skill, and attitudes to provide care that is based on cultural humility/safety and is free of racism
- Ensure timely and appropriate identification, triage, and treatment of people presenting with vaso-occlusive acute pain episodes
- Create or adapt digital order sets for acute vaso-occlusive crisis/SCD care management
- Embed care plans into the hospital information system where applicable
- Support patients and families in transitions in care, including appropriate referrals to SCD centres where applicable
- Collect data to measure quality standard adherence & improve SCD care processes overtime
Resources Available for Partnering Sites
E2P provides resources and support to guide hospitals through an implementation. Participating sites will work closely with the E2P Program and OH to drive and accelerate the adoption of the quality standard through digitization, supporting quality improvement efforts. These resources include:
- Funding provided directly to hospital sites to compensate resources required for implementation (amount TBD)
- Project management support with planning and executing all phases of the project
- Clinical informatics consultation to support facilitating clinical and technical discussions
- Change management support in creating, tailoring, and disseminating change management materials to meet individual sites’ needs
