This tool is designed to help clinicians identify, assess, diagnose and manage psoriasis. While there is no cure and complete clearance may not always be possible, treatment can effectively manage symptoms. Regular follow-up is essential to monitor response to treatment and disease control.

About the tool

This clinical tool is designed to guide discussions and decision-making between clinicians and people living with psoriasis. It aims to support primary care clinicians with:

  • Recognizing psoriasis presentations (subtypes, body sites) in different skin types (including skin of colour), and in assessing its clinical severity and psychosocial burden.

  • Using clinical judgement to guide pharmacological treatment selection based on disease severity, body sites involved, and shared decision-making for patient preferences.
  • Engaging patients in shared decision-making and sharing resources to support lifestyle strategies for long-term management.
  • Follow-up and monitoring of patient response to treatment based on shared goals of treatment.

The Psoriasis tool was developed using the CEP’s integrated knowledge translation approach. This approach ensures that clinicians are engaged throughout the development processes through the application of user-centered design methodology. Clinical leadership was provided by Dr. Jessica Howard. Leaders from the primary care sector, dermatologists, key organizations, persons with lived experience and end users were also engaged throughout the tool development process to provide feedback and support the tool’s refinement. 

The tool is among several clinical tools developed as part of the Knowledge Translation in Primary Care Initiative. Funded by the Ministry of Health, this initiative supports primary care providers with the development of a series of clinical tools and health information resources.
